How to Refer an Adult to Heroic Path’s Partial Hospitalization Program

Heroic Path welcomes referrals from therapists, physicians, dietitians, hospitals, and other healthcare professionals seeking structured, trauma-informed care for adults with eating disorders and related concerns.

Our admissions process is designed to be collaborative, responsive, and clinically thoughtful, with the goal of supporting appropriate level-of-care placement for each individual.


Who May Be Appropriate for Referral

Heroic Path’s adult Partial Hospitalization Program (PHP) may be appropriate for adults who:

  • require more support than traditional outpatient therapy
  • demonstrate increasing eating disorder behaviors
  • experience significant emotional dysregulation
  • present with co-occurring trauma-related symptoms
  • are medically stable but need structured daily treatment
  • are stepping down from residential or inpatient care
  • have plateaued in outpatient treatment

When there is uncertainty about level of care, our team is available to consult.


Referral Sources We Commonly Work With

Information Helpful at the Time of Referral

While not all materials are required to initiate contact, the following information can help expedite the review process:

  • brief clinical summary or referral note
  • current diagnoses (if available)
  • recent height/weight or medical information (when relevant)
  • medication list
  • insurance information
  • prior treatment history
  • safety considerations or recent risk concerns

If some information is unavailable, referrals can still be initiated and our team will assist in gathering needed details.


What Happens After a Referral Is Made

Once a referral is received, the Heroic Path admissions process typically includes:

  1. Initial intake review
  2. Clinical screening to assess level-of-care fit
  3. Insurance and benefit verification
  4. Coordination with the referring provider (as appropriate)
  5. Scheduling of admission when clinically indicated

Our goal is to provide timely feedback to referring professionals and maintain continuity of care.


Ongoing Collaboration With Referring Providers

Heroic Path values partnership with outpatient and community providers. When appropriate and with proper authorization, our team may:

  • coordinate treatment planning
  • provide progress updates
  • support step-down planning
  • assist with continuity of care after discharge
