Heroic Path is committed to supporting members beyond the Partial Hospitalization Program through thoughtful discharge planning and coordinated aftercare support. From the time of admission, the treatment team works collaboratively with each member to identify appropriate next steps that promote stability, skill generalization, and sustained recovery.
For many individuals, step-down care may include outpatient therapy, structured groups, nutrition support, or community-based services, depending on clinical needs and recovery progress.
Heroic Path maintains a formal linkage agreement with Hugh S. Smith, Ph.D. & Associates, P.C. an affiliated outpatient practice located within the same building. When clinically appropriate and aligned with member preference, individuals may transition to HSSA providers for outpatient therapy and group support following discharge from PHP.
Experiential groups available through HSSA may incorporate modalities such as art-based interventions, movement and music experiences, and horticultural activities designed to reinforce emotional regulation, embodiment, and real-world skill application beyond the PHP level of care.
Heroic Path also maintains collaborative relationships with additional community providers and higher or lower levels of care to ensure that each member’s discharge plan is individualized and clinically appropriate.
